Root canal had broken file extending out of the apex. Root end surgery was completed to remove broken file and root end granulation tissue. Patient came in with an apical perforation and periapical radiolucency at the apex of mesial. Root end surgery was completed on the mesial roots and bone graft was added. 6 months later, the surgical site still looks good and patient was out of discomfort. Broken File Bypass number 3
